? Care for hospice patients in private homes and/or healthcare facilities ? Assist with activities of daily living, such as personal hygiene, assistance with meals, ambulation, repositioning, etc.
? Implement plan of care under the directive of a registered nurse ? Use safety equipment, as trained, to reduce risk of injury to patient and self, as instructed by plan of care and in communication with nurse concerning patients' equipment needs ? Perform light housekeeping in patient area only, when needed, including changing bed, laundering of bed linens and patient items ? Document patient care in electronic software using cell phone ? Use telephone in a professional manner, as needed ? Ability to travel to patient homes and facilities as assigned Other responsibilities as assigned Certified Home Health Aides are needed for all counties in South NJ.
Requirements CHHA - Certified Home Health Aide - HHA - Inpatient Center Aide - Inpatient Hospice Unit HHA Must have current NJ HHA license.
